Medical Assistance: Home Care Programs

These programs are designed to provide personal and supportive health care services to individuals in a safe, home-based environment.

Consumer Directed Personal Assistance Program allows individuals to manage their own personal care needs by hiring, training, and supervising their own personal care attendants.

The Long Term Home Health Care Program, coordinated with the Department of Health, offers an alternative to nursing home placement by providing skilled nursing, home health aide, therapies, home-delivered meals, and personal emergency response systems.

The Care At Home Program provides skilled nursing services for disabled children under the age of 18.

Disabled Adult Child Waiver (DAC) Social Security benefits are received upon the disability, retirement or death of a parent.

The Nursing Home Transition and Diversion Waiver (NHTD) uses Medicaid funding to provide supports and services to assist individuals with disabilities and seniors toward successful inclusion in the community.

The Personal Care Program and Limited Licensed Home Care Services are programs that provide personal care for individuals.

Eligibility:

Referral process:

A referral from a primary physician needs to be submitted to either DSS at (315) 366-2211 or the Department of Health (315) 366-2361 or an individual may call personally to discuss which program is best suited for his/her needs.

Once a physician’s referral is submitted, the DSS home care coordinator, along with a public health nurse, will schedule a home visit for assessment and approval.
